Are you passionate about using technology to improve people's lives?
We are looking for a Technology & Digital Inclusion Social Prescriber Link Worker to join GRCC's award-winning DAISI Project team, working alongside GP practices across the North Cotswolds to support patients who face barriers linked to isolation, long-term conditions, disability, mental health, or digital exclusion. You will work to help them access practical human and technology-enabled solutions that improve their lives.
You will:
work directly with patients referred by GP practices and partners;
co-produce personalised support plans using digital assistive technology;
help people to build confidence with devices, apps, and online services;
connect patients to heathcare, community, and voluntary support;
champion inclusive, accessible technology that works for everyone.
The successful candidate will be:
good with people;
patient and able to listen;
confident using everyday technology, with the ability to explain things simply;
passionate about reducing health inequalities and digital exclusion;
interested in working across the healthcare, community, and voluntary sectors.
