GRCC is looking to recruit a Social Prescribing Link Worker for the North Cotswolds

Social prescribing empowers people to take control of their health and wellbeing through a referral to non-medical 'Social Prescribing Link Workers' who give the time and focus on what matters most to an individual's health and wellbeing, connecting them to community groups and statutory services for practical and emotional support.

We are now recruiting for a full-time Social Prescribing Link Worker (SPLW) to join a growing team of Primary Care Network (PCN) healthcare professionals and the Cotswold Community Wellbeing Service working across the five practices of the North Cotswold Primary Care Network.

For this Adult Social Prescribing Link Worker role, we need an individual who can harness their skills and holistic approach to supporting individuals to be a pro-active SPLW, working with communities to build support and create new pathways / activities, working with partner organisations to achieve this.  You will work with a diverse range of people and local communities, to draw on and build on their strengths and capacities, enabling local voluntary sector organisations and community groups to receive social prescribing referrals, nurturing community assets and supporting them to become sustainable, as well as identifying gaps in local provision and creating new social opportunities to help fill the gaps.

Providing personalised support to individuals, their families and carers, you will take a holistic approach based on the person's priorities and wider determinants of health, working with the client to co-produce a simple personalised care and support plan to improve health and wellbeing.

You will work with direct supervision of clinicians, taking referrals from the PCN's five Core Network Practices and from a wide range of agencies, whether public or voluntary sector, and be part of a wider supportive team based at GRCC.

We're looking for enthusiastic and passionate candidates with excellent partnership working and listening skills, with a 'can do' attitude.

You may have experience of supporting people, their families and carers in a related role, including supporting people with their mental health, and of working with the VCSE sector.  You may have had experience of volunteering yourself and understand the principles of Asset Based Community Development. 

We operate a hybrid way of working and you will be based in GP practices, community venues, and will work from home or at the GRCC offices in Gloucester.  You will do face-to-face visits as well as supporting people via the phone, online, or out in the community.
