Have you served in the Armed Forces?  Are you interested in supporting ex-forces personnel with their health and wellbeing across Gloucestershire?

The main purpose of the role is to help develop a Social Prescribing service for Ex-Forces personnel, their families and carers across the county.  As part of the Stroud and Berkeley Vale Community Wellbeing Service, you will undertake assessments of need and help to develop a  robust and holistic service for individuals.  You will achieve this through finding and working directly with ex Armed Forces personnel, ensuring that they are receiving the full support they need through appropriate medical care, connecting them to local community resources and identifying skills that could be utilised in engaging in activities or facilitating events for other personnel.

You will also build strong relationships with partner organisations including Armed Forces specialist support, statutory services, Primary Care and Voluntary & Community Sector partners and the Department for Work and Pensions (DWP).  You will work on a small case load developing individual action plans and empowering individuals to self-manage and take control of all aspects of their own personal health and wellbeing.  In addition, the successful candidate will be responsible for supporting the education and training of existing Social Prescribers across the county around the needs of the Ex-Forces community.

Social prescribing enables GPs, nurses and health professionals to refer people to a range of local, non-clinical services to support their health and wellbeing through a range of activities including volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.  Individuals and their families and carers can also refer directly into the service.
